Health Evaluation Form for Early Childcare Qualification (Colorado)
One of the required steps for ECE substitute qualification with HelloSubs is a Health Evaluation Form. The health check is a standard medical evaluation form to be completed by any health care professional.
There are a few different ways to fulfill the Health Evaluation requirement:
1. If you have already completed a Staff Health Evaluation form for the CDEC or another ECE provider within the last 9 months, then the same form can be used for HelloSubs. Upload a copy of the completed form to the Onboarding Portal.
2. If you do NOT already have a signed Staff Health Evaluation, HelloSubs recommends booking an online appointment with our preferred provider Colorado CPR Pros. This appointment is entirely online for convenient booking and quick results. You can book your online appointment here: https://www.coloradocprpros.com/medical-physical-statements. Following your appointment, Colorado CPR Pros will send the signed evaluation. Upload a copy of the completed form to the Onboarding Portal.
3. For an in-person appointment, download and print the Staff Health Evaluation form. This can be filled out by any healthcare provider:
- Primary Care Physician
- Clinic
- Any healthcare facility that provides routine physicals
Many Pharmacy and Grocery Store chains offer quick and affordable employment physicals to complete this process. There are many options available, but HelloSubs recommends The Little Clinic at King Soopers.
**NOTE: The Staff Health Evaluation must be completed annually. For more information on document expiration and renewal, please review this article:
