Health Evaluation Form for Early Childcare Qualification (Pennsylvania)
One of the required steps for ECE substitute qualification with HelloSubs is a Health Evaluation Form. The health check is a standard medical evaluation form to be completed by any health care professional.
There are a few different ways to fulfill the Health Evaluation requirement:
2. If you have completed a physical/Health Evaluation for employment/school/any other reason within the past 12 months, this may also meet the requirements. The health evaluation should include a note from the physician that the patient is cleared for employment duties and activities, and should also include a TB Test. If you have such a document, send to HelloSubs to verify if it is sufficient to fulfill the Health Evaluation requirement.
3. If you do not have either of the above, then you will need to have a healthcare provider fill out the Child Care Staff Health Assessment form. This can be filled out by any healthcare provider: - Primary Care Physician
- Clinic
- Any healthcare facility that provides routine physicals
Many Pharmacy and Grocery Store chains offer quick and affordable employment physicals to complete this process, such as CVS or Rite-Aide.
