1.  Fill out this Reference Contact Form (Recommendation Letters are also accepted).  **Please note that a minimum of 2 references are required
   
2.  Please sign the following forms:  
   
1.  The HelloSubs contract (sent to you in a separate email).  
   
2.  The National Sex Offender Registry Verification.
   
3.  The Pennsylvania Department of Education ACT 168: Employment History Review Form. 
   
3.  Complete the HelloSubs New Hire Orientation.  
   
4. For candidates that qualify for Assistant Group Supevisor/Group Supervisor without a relevant degree, you will need to verify having 2 (or more) years of experience with children.  Please email your recruiter the contact information of the relevant previous employers that can complete the DHS Childcare Evaluation form.  These forms will be completed by your previous employers to verify your experience.  Your recruiter will send through the forms through Zoho Sign.  
   
   

Upon completion, please upload the following to your Onboarding Portal:  

1. A picture or scan of your highest degree. If you don’t have access to your degree, the transcripts will do.  
   
2. Complete your Child Abuse clearance.  
   
3. Complete your Pennsylvania Access to Criminal History clearance (PATCH). 
   
4. Complete the PA Early Childhood Education Trainings.  
   
5. Complete Fire Safety Training
   
1. This training must be done in-person or online through an approved provider.  To find and register for a course, please search for "Fire Safety Training" using this link here

In-Person Screening Requirements:  

Upon completion, please upload the following to your Onboarding Portal: 

1.  Schedule and complete your FBI Fingerprint clearance.  
   
2.  Download, print, and get the Health Check form signed by a health care professional.
   
1.  Have you had a health assessment completed within the last year? If yes, simply upload it your Onboarding Portal.

3. Complete Pediatric CPR Training
   
1. For a list of approved providers, please follow this link
   
2. While an Adult CPR Course is beneficial, there must be a Pediatric Component to the course in order to qualify 

Additional Screening Requirements: 

1.  To be completed within the first 45 days of hire: complete the Mandatory Reporter Training.  
   
2.  Have you lived outside of PA within the last FIVE years? If YES, complete your out-of-state background check.
